NAME/HANDLE: Your name.
CHARACTER: Your character's name.
USERNAME: Their username.
HOW IS YOUR CHARACTER LEAVING: Are they going to be killed off by a Witch? Are they turning Witch? Are they just quietly being subjected to the whims of the hole? Let us know. If your character is going Witch, be sure to make an ooc post stating so and outlining their barrier and Witch form.
If you're going on a break, the process is the same as above.
